How do i sucessfully find the fort atkinson DPS office's Fax Number? Need to fax in a dr. Note to your office for missing work today. I also need to send another Dr. notice to Cindy Burche the supervisor of/for the Janitorial crews. Shes my supervisor and i wld like to fax a dr note to her office as well for missing work today.So quick recap: I need the fax number for the DPS Fort Atkinson office and the Fax Number for my janitorial supervisor Cindy Burchki as well so i can have a dr. Note faxed over to youre DPS office in fort and i need the fax number to send a dr note via fax to my janitorial supervisor Cindy as well, for missing work today. I had good reason. If you cld get me either fax numbers i wld be very greatful for that. I had a very good reaaon for missing work.
