How do i sucessfully find the fort atkinson DPS office's Fax Number? Need to fax in a dr. Note to your office for missing work today. I also need to send another Dr. notice to Cindy Burche the supervisor of/for the Janitorial crews. Shes my supervisor and i wld like to fax a dr note to her office as well for missing work today.So quick recap: I need the fax number for the DPS Fort Atkinson office and the Fax Number for my janitorial supervisor Cindy Burchki as well so i can have a dr. Note faxed over to youre DPS office in fort and i need the fax number to send a dr note via fax to my janitorial supervisor Cindy as well, for missing work today. I had good reason. If you cld get me either fax numbers i wld be very greatful for that. I had a very good reaaon for missing work. This is the second time I got fired for no reason. I was told they need someone more reliable and thought I didn't go to work. Told them to check my time card. The person kind of chuckled and said looks like we accident fired you and never gave me another job! Very unprofessional what they did. Years later I came back in need of a job, and they told me I was going to have a position just need to train for it. I had told one person what my position was going to be and they asked the supervisor if I was the assistant supervisor and he said no. Obviously there is no communication between people. Point being the supervisor told me his loyalty is with the people he had now. He wanted his "best guy" to have my position. I got home the same day and received a call the my assignment ended.
